There are no copies of the form left in the hide, just an email address for Rachel on a small poster stating that 'permission' will be granted after a request to Rachel is made.

You need to fill out a "risk assessment" form and send it to Rachel Graham @ NE Lincs Council. Paper copies of the form are available in the hide at Cress Marsh.

Alternatively, if you email either Rachel or Sian Niblo, they may have an electronic version of the form that you can complete and email back to them.
